I have an instance where a 18 years old, HSP 132 kV outdoor bushing failed.

It comprised of a glazed porcelain column built up from four segments, which were said to have been epoxy cemented together. The bushing failed at a midpoint of the second to top segment. This was said to have been the second of two similar failures at the same site.

Has anyone else had experience of failures of this type of bushing. If so please tell me if the failure was attributed in any way to the unusual (to me) design.
