Does anyone have a reference for converting HS-25 loading to an equivalent uniform load for comparison of loads used to design a slab on grade?

Use the AASHTO lane load of 640 plf over a 10' lane and multiply by 1.25 to convert the HS20 load to HS25. The only thing left is whether to include the concentrated load associated with a lane load. However, shouldn't you be using an ESAL for pavement design?

You can't expect to design a slab on grade for uniform loading, as the critical loading is always point loading or wheel loading. Using uniform load equivalents can work for suspended slabs, but not for slabs on grade.
