Black Sulphur production

Black Sulphur production

(OP)
Hi all, I work in a refinery Operations division. We lost the SRU on high pressure due to blockages in condensers caused by production of black Sulphur. We clearly had a hydrocarbon carryover with our acid gas feedstock (Normal composition 95% H2S, 4% CO2 & 1% HC less C4) from the amine treating unit. Before the regenerator there's a flash drum installation, 2 phases, amine and flash gas routed to fuel gas operating at 3.5 bar.

Need much guidance to get to the root cause:
Hydrocarbons are soluble in amine, historical trends don't indicate an increase in off gas prior to the incident, so how did they get to the regenerator then SRU?

Amine storage, lean and rich amine sample are clear color after skimming the tank, no sample analyzed unfortunately. Currently shake test is not forming and color is clear indicating normal.

What other safe guards or installations are available to prevent hydrocarbon to SRU?

Would much appreciate some guidance on finding source, what failed and how to prevent reoccurrence.
