I had to "dabble" in some mechanical design and check flange sizes and rods/bolts for a 54" DIP main at a valve gate as part of a potable water system.

Do these components generally see a cyclical loading that would require fatigue to be checked?

Every time it is opened or closed.

Independent events are seldomly independent.

If the applied stresses are less than the fatigue limit of the materials, the "check" probably becomes much easier, or less dependent on guesses of numbers of cycles etc.
