Geotechnical Boring Assignment Sheet

(OP)
Due to some communication issues that have been occuring betwenn our firm's engineering and drilling staffs regarding geotechnical jobs, I am considering coming up with a Geotechnical Boring Assignment Sheet to use for each job that can be given to the drillers with job specific requirements. Something with project information, due dates, when drillers should contact engineers (such as schedule drill date, found fill, etc.), when to sample, what to do with borings when complete, etc. I think I could come up with something, but surely someone has something similar to this? Curious if anyone had experience with something of this nature and if there were any examples out there I could find?
