Does API 650 require a manway cover / adapter to be made from one solid piece of plate? Can it be two pieces (say, an inner and an outer section) that are bolted or welded together?

I'd have to look at the exact wording to see, I suppose. But it's not uncommon to have mixers that are built into or bolted onto manway covers, although some additional thickness is required in that case.
