ASME IX QW-288.1 says "A change in the weld joint configuration (beyond the manufacturing tolerance) such as the addition or deletion of preplaced filler metal, an increase in the depth of the groove, a decrease in the groove angle, or a change in the groove type."

Please help me to understand that what is the criteria to determine beyond the tolerance limit?

Increase in depth of groove, does it mean if I have a WPS qualifies on 3mm grove depth I am eligible to weld from 1 to 3mm groove depth but not more then that?

Say tube to tubesheet joint is groove plus fillet welded (see figure UW-20.1, sketch c or d).
Drawing shows that groove is V-Type (45 degree, say) and depth is 3 mm plus 0.2 and minus 0.1 mm.
Now, you qualify PQR with 3 mm groove depth. The qualification range would be as follows:
groove depth: 3 mm max. (but you can go to 3.2 mm because you are within the manufacturing tollerances)
Groove type: V, 45 degree minimum, if drawing shows 45 degree plus minus one degree, then you can even go to 44 degree because you are within the manufacturing tollerances.
