IPW collision above the part?

In my setup, the MCS is on the top of the part, the blank is auto block with 0.5 mm Z+ stock. When I verify with 2D dynamic, tool holder and IPW collision check ON, there are collisions, but very weird, because the Z coordinate of the point +4. Why?
Cavity mill operation in NX 8.0.3.4., Win7x64.
There are other strange things. When I simulate like above, but two times, it gives different points as result for the same path.

This is probably best handled if you log a call with GTAC and let them look at your case.
