Twisted bend in sheetmetal

I have a small tab to model in solidworks, it has a bend about half way along its lenght and is also twisted where it is bent. I have tried to loft it and use the sheet metal tools but nothing quite gives me the right geometry. I'm using solidworks 2008, if any one has any tips I'd be greatful. The origional drawing is attached and might make clear my problem.

Sketch should be at an angle to top of tab
Model is sheet metal part to length (6") width (2") and thickness (10Ga)
On part shetch I added a linked value for bend angles (Twist)
Part sketch has unequal legs to return to parallel after bending
Added sketched bend at an angle using 1" y difference in heights
Added a second sketched bend at an angle using 1" y difference in heights on opposite face
Linked bend angle(Twist) in both sketch bends
Added sketch to verify twisted angle
Edit (Twist) as required
Second sketch bend is not necessary but typical
