There are places on the web which state that under 4 inch NPS API 5L pipe can not be ordered to PSL-2. Please confirm that this is incorrect. One of these places is: http://wiki.answers.com/Q/What_is_the_difference_b.... Thanks!

It's too small to cut out proper test samples.

That is certainly true and then note at the end of section 10.2.3.3 exempts pipe not listed in table 22 from CVT which results in pipe less than 4" NPS not being subject to CVT. But nowhere in the rest of the spec is there a size limitation that I can find.
