Concrete Floor Slab Core needs to be plugged

I am rearranging equipment and will have holes in a concrete floor that need to be pluged with concrete or epoxy grout. The holes are approx 4" or smaller diameter. We have details to put a 4" angle under the floor and epoxy anchor it into the underside of the floor for form work and then fill the hole with concrete. The question has come up that if there is limited or no access under the floor for workers, what can be done? A brief internet search is not bringing me the type of products I have expected for this situation of forming a hole plug from the floor above. Does anyone know of a product or procedure for forming the bottom of a hole for a small core cole repair? Thanks is advance.

or
Take a plastic coffee can lid - put a hole in the middle and a skinny bolt. Stuff into hole and pull it into place.
Do your repair.
