NASM 1352 notes 4 finishing details (cadmium plate, silver flash, etc) for Heat resistant materials. Are all of them applied or does the user specify one of the four?

No, all four do not apply. The end user specifies which one to use. I don't have this particular standard right in front of me, but there should be an explanation of how to specify the finish. Usually one of the characters that appears after the dash:

NASM1352-???
