can anyone point me to a website, or example circuit that does the following:

Controls a dc motor via a 0-5VDC proportional input signal.

Please, provide more info about your motor:
1. Linear or rotary
2. HP or kW rating
3. Voltage rating
4. Application

you may select a standard DC drive from any reputed manufacturer like Siemens, ABB etc which can easily accept 0 to 5 VDC analogue signal inputto control speed of the motor.
