Medical Device Instrument with Premature Corrosion

(OP)
The instrument is made of 420 SS and has an TiAlN coating applied.  It has been in use for approximately three months and is cleaned by autoclave after each use.

Take at the attached photo for the visual.  Any assistance would be much appreciated.

The wash it before autoclave, and they are leaving residue from the cleaner, or it is still wet.
The coating may look good but it does not provide any real corrosion protection.

The coating is for appearance only, was thinking perhaps the temp. may have affected the material (only I don't know the temp at this point, but should know tomorrow), also don't know the condition of the SS.  Finally, I failed to note that the device was passivated (citric)
