I have posted this topic before with little luck.  I am curious to see any instances of failures of old vessels constructed with A212 material.  The only record I can find is the following rail car.

I recall a vessel that failed during a rehydrotest, either required for an alteration or rerate; I forget which. Unfortunately, the records of such failures are often unavailable because of litigation and their settlements being sealed or the Owners of the equipment keep their records secret. You might try researching OSHA for such records where injuries occured.
