Mixing Point Problems in Refineries

I am reviewing potential "mixing point problems" in a refinery.  I have been told that there have been two - three significant problems recently in refineries, due to either low/high temperature mixing points or low/high pressure mixing points.  

Erosion due to mixing problems has created sudden and significant leaks.

Does anyone know of papers in circulation about these recent problems ?  I could name the refineries, but probably that is not best to do so.
