Can anybody provide me the published/issued Code Case & Interpretation related to "HG-701-MOUNTING SAFETY AND SAFETY RELIEF VALVES" - ASME IV?

The code case is #2658.  It says it's permissible to mount the safety relief valve on the hot water end of a intermediate header providing that six different conditions are met.  However, the document is copyrighted and I don't believe I can paraphrase the six conditions without stepping over the line (they're pretty detailed).
