In which size of pipes or velocity of fluid should use thrust block on elbow?

What fluid's mass?   Slurries can give a lot more thrust.
What kind of pipe?   Welded pipes usually don't need them.
What kind of flow?   Steady state might not need it, transient pressures might.

And is there a Positive Displacement pump in the pipe circuit?  Without a Pulsation Dampener, those pumps pound pipe hard.

May I add to Biginch question list the following:
What pressure?
What deflection angle (elbow angle)?
what cover height of the pipe?
restraint or unrestrained joints?
