How do I go about creating an extension of a radius to dimension to? Is this a drawing option somewhere?

The easiest thing to do is create a note with the dimension value inserted parametrically:  R&d###

I've asked PTC to do this about 20 times, they never sweat the details, they would rather hose the complete user interface.

How do I go about making the extension though? The note just attaches to the curve. I should clarify I'm on WF3.

I didn't really grasp what you wanted to extend.  I thought you wanted to have a line under the text.  You want to extend the arc of the radius.  Just pick right at the arrowhead.  You will see a little rectangle.  Drag it around as you need.
