1.Seeking further advise and recommendation.
   a.    Do we need to replace the affected items?.
   b.    If NO, is there any way forward to ensure that the integrity    of the affected items is intact.

This system is so poorly designed, why even think of operating it any longer.  Cut it all out, design it properly and this time make sure that you include the necessary pressure control and relief valves!

Is that piping actually bulging?  It sure looks like it from the drawing.  If so, was that a manufacturing defect or did the "pressure excursion" cause the bulge?

My vote is to cut out and replace with a properly designed system.

But, you're the engineer and "somebody on the Internet told me to do it" is probably not going to be acceptable to your boss, especially if it involves money.

What was the original Code of Construction? What's the yield stress of the material and was it exceeded?

I see some NDT has already been done.  Along with that, would a hydrotest help determine it's acceptance, or not?

Jeez. 60 bar in a 150# system. Says alot for conservative design rules. My advise .... if you don't replace it, stand clear.

And follow Big's advice regarding a PSV.

It's fortunate that it didn't fail...I'm suprised.

I would definately replace, simply based on the fact that the material was obviously overstressed.

That part of the system needs to be protected from wherever that overpressure came from, or even better yet, designed for ANSI#600.

All I can say is that you'll damn lucky nobody was killed!
