Does Machining Flanges from Bar Stock are permited in ASME VIII - Div 1?
If yes - Code Reference / Code cases?
If no - Code Reference / Code cases?

Look at the correct ASME specification for your answer --------

ASME B16.5  Pipe Falnges and Flanged Fittings

Once you have a copy, look for the material group and find the ASTM specification that is applicable.

Perhaps you should look up on UG-14 of ASME VIII-Div 1 for limitations on the use of bar and rod material for flanges.

As far as it concern to a Hubbed Flange, machining it from a bar stock material is prohibited by mandatory Appendix 2 unless the requirements of 2-2 (d) (2) are met.

SA-105 and SA-182 clearly specify that it is not allowed.

The OP question was regarding the use of bar stock material per ASME VIII Div 1 and was not related to any particular specification of forged materials. Also, nowhere in the post was mentioned ASME B16.5 flange or any other specific type. It was simply asking if bar stock material can be used for flange fabrication in accordance with the BPVC. The short answer is yes, in accordance with UG-14. Perhaps the answers should stick to the point or mention the deviation from the question, in order to not confuse the OP.
