I have an assembly with a block in it.  In that block are 3 holes for hydrualic cylinders to mount.  With the assembly open, I double clicked on the block to activate it, created a sketch, and then extruded a pocket for access to the backside of two of those cylinders.  

Once I make my assembly active again, all constraints associated to the block have lost one piece of geometry (yellow exclamation point).  So, I have to go into each constraint and reconnect.

What did I do wrong???

I'm using V5R20 SP2.

Some questions to clarify your question:

Assembly constraints or sketch constraints?

How did you save the block after you modified it? (did you rename it?)

If the assembly constraints got disconnected, did you constrain to the face that you removed the pocket from?

I went into Save Management and did a "save" not "save as".  So, no, I did not rename it.

If the assembly constraints got disconnected, did you constrain to the face that you removed the pocket from?  Yes, I had one constraint in the face that the sketch/pocket was on.  However, the two cylinders on a different face, and the block to the base plate constraints were also lost.

Shortly there after, I made another cut on a face with no constraints, and had no issues.

The extruded pocket (??) did it affect an element used for the constraintment? was a face consumed/divided?

  No, I just notched a corner.  I've been watching myself a little closer, just to make sure, but I haven't had the issue again.

I can imagine that notching that corner consumed the edge, so if that edge was used for contraintment then it would result explanation mark that needs to be re-routed. But you also affect four surfaces that shouldn't affect anything... do you know what kind of constraint element was missing?
