I am trying to find information on designing a split collar/shaft collar.  I am looking for

1. ID tolerances of shaft collar in comparison to the OD of a shaft.
2. OD recommendation of shaft collar and bolt size ect.

1.  A little bigger.
2.  Any damn size you want, since you're designing a custom part instead of using a commodity part.

2a.  The OD should be generally larger than the ID.

Enough gap so it will clamp.

ok yes that is all true but im looking for something that will give recommend practices for designing this.  

ex. "Enough gap so it will clamp", ok yes that's obvious but there as to be a rule of thumb like for "X" diameter should have "Y" gap. Or "X" clamp force should have "Y" gap.

Any positive gap with the bolts torqued will do.  ... unless you need it to appear complete, e.g., with no visible gap, in which case you crack the part.

Why would you want to do this?  What part of the gazillion off the shelf split collars doesn't work for you?
