How do I reverse a 1PH motor with relays?

I am looking for a wiring diagram that uses two relays with inputs from a PLC that would reverse a single phase gearmotor. The gearmotor will open and close a door with a limit switch at each end to stop the door when it is at the end of its travel.

Pole #1, Normally Open,- Connects orange to Capacitor when energized.
Pole #2, Normally Open,- Connects yellow to brown when energized.
Pole #3, Normally Open,- puts power to red or black when energized.
Relay #2, double throw, triple pole.
Pole #1, Normally Open,- Connects yellow to Capacitor when energized.
Pole #2, Normally Open,- Connects orange to brown when energized.
Pole #3, Normally Open,- puts power to red or black when energized..
Do not use the normally open and the normally closed contacts on any one pole. There is a danger of flash over between normally open and normally closed in some usages. If you can not analyze your circuit for hazards, just don't try to get double duty out
of a double throw pole.
Program a slight time delay when switching directions.

I am not an electrician, so your description is a little confusing, especially the capacitor connection. I have attached a link to an image of the motor and a reversing contactor.
Is it possible to print the image and sketch in the wires and fax it to me? If you can't no problem.
I would hate to wire it the wrong way and hurt the motor or me. My fax is 773-463-9222 attention Paul.
