If an operating pressure vessel needs to work at a higher temperature and lower pressure than the originally design,
what's the right procedure to re-rate the pressure vessel?

You have two options;

See the national Board Inspection Code, NB-23

OR

API 510 Pressure Vessel Inspection Code:
In-Service Inspection, Rating,
Repair, and Alteration

Also check your Jurisdiction requirements.

As stanweld notes... check your local requirements.  The above would probably mean something in America but mean nothing anywheres else.
