I can set an associative dimension origin, after I've placed it, but where's the setting/preference for associativity during the creation ? I know there used to be a setting.

As you're placing a Dimension, look at the right-end of the 'Selection Bar' and you will note a set of icons which will provide access to the various annotation placement options including controlling whether it's to be defined as Associative or not.
