I have a question for structural engineers out-there familiar with connection design.  I am using FEMA 350 to design a moment connection with top & bottom flange plates.  One of the steps is to check block shear, including bolt pull through (Fig 3-19d pg 3-59).  This would be where tension in the flange plates causes all of the bolts in each line (parallel to the beam flange) to unzip, as it were.
My question is that if the failure plane is down the bolt holes from the farthest away to the edge of the beam or plate, this failure surface is in shear, but for each line of bolts do you have one shear failure plane or two?

LRFD second edition, section J3.10b, I understand that a single shear failure of the base material will only control when the minimum spacing of bolts and min. edge distance is not met.
 (represented by Equation J3-2A)
 
Otherwise I think bearing, yield across the gross area, fracture on net (across member perped. to force), shear on bolt, and block shear are the concerns.
