we have a small project where the customer wants a welders log on a spreadsheet. Does anyone have a simple example?

It can be as complicated or as simple as you need to make it.

  We use weld mapping and tracking separate from our welder logs.  Our welder logs are used primarily for maintaining welder continuity and to assist with HR functions.  A separate log helps rod room attendants verify that the welder has the necessary qualifications to have the requested filler issued to him/her.  

If you can better define what the customer is looking for, we can probably offer something a bit more insightful.

Some question to ask: is NDE involved at all?  Are your welders assigned stencil numbers?  Are they assigned employee ID numbers?  Are they qualified on different materials with different fillers?  Are field weld numbers assigned to your weld joints, or is the welding primarily structural? etc.

Thank you for the replies. We are an ASME certified shop so all our welder have their stamp# etc. and there is only shop work.  But this project is a huge light gage duct work that is not even ASME but the customer wants a welder log. He wants a huge mountain of paperwork and of course my boss wants a simple document. I am trying to get the middle ground. We have had other requests for welders logs so I was trying to find out if there is an accepted standard that I can start with. Metengr's example looks as simple as possible so I guess I can start with that.
