if the FILE and INPUT parameters are omitted,
then the data lines that follow are like this:

node number, value1, value2, value3

What are these values?
The new coordinates of the node?

They are not the new coordinates - they are the distance that the node is moved in the coordinate system specified.  

Check the section in the Analysis Users Manual - section 11.3.1 "Introducing a geometric imperfection into a model" covers the options that are available.
