Hello Kind Sirs:
I am looking for a 40-50 shore soft plastic or rubber material supplied in 1/4 in. sheet for cushioning a prosthesis, that said material could then be softened (only softened not completely melted) with heating elements embedded in it to for a final shape under load.  It of course cannot give off toxic gases like PE's and PU's.  Any help would so be appreciated!!!

You might want to consider fibrous padding. Synthetic fiber felts can be heat set to shape and to different densities so the hardness can be controlled. They also breathe in contact with skin. I recall reading an interesting tech article about the advantages over foam for automotive seat cushions.
