When is it necessary to use an epoxy grout rather than a cementitious grout.  What forces acting upon the grout bed would require an epoxy grout?  How does the compressive strength of the epoxt gout account for the creep (cube deformation) experienced during testing (ASTM C 579)

The choice depends on your particular application.  For instance, epoxy is the preferred choice when used for supports on or near vibrating equipment/piping, or in the presence of a dynamic load.  Epoxy of course is the better choice when certain chemicals are likely to come into contact with the grout (such as lube oil).

In static, simple load bearing situtations (such as building columns), cementitious grout is acceptable, and more economical.

I don't know that there's a specific calculation or value that you can use to help make the determination between the use of one or the other.
