RJ45 is 8P8C (often used for Ethernet), and RJ11 is 6P2P (often used for POTS). What's the application? If you're using Cat 5/6 wiring for POTS, most installers would simply reterminate. If you must have an adapter, you may have to make an adapter cable (with dual female couplers as required).
