Are there any known standards specifying concrete plant inspection requirements during superstructure operations? Is there any state (NY) or national codes outlining specific requirements to have an inspector(s) at the concrete batch plant? Any help on this would be greatly appreciated.

Use the National Ready Mix Concrete Association's checklist.

As for mandating such inspections, I don't know of a code mandate...it is usually specified by the architect or the structural engineer if such inspections are desired.
