I am looking for some input on how others perform LOTO on Hansen 3-way valves and Vilter stop/check valves.  The valve stem is all there is and is acceptable to drill the stem to attach a locking device?  Or, does the operator put the valve in the closed position and put one of those external devices that clamp onto the piping?  Any other means of LOTO?

Do not drill the valve stem, ass anyone who is intent on defeating the system won't let a small inconvenience bother him.  As you stated there are external device that can lockout a valve and wouldn't affect the integrity of the valve.
Be careful in selection of any device, keeping in mind how the valve is to used.  One way we do this is to chain the operating handle or wheel, either in the opened or closed position.
