Do you normally include entrance loss of a vessel nozzle in the PSV inlet piping pressure drop calculations?

I normally do, however, I see other engineers that only include the piping from the vessel nozzle flange to the PSV.

Yes.  Pay attention to/specify the nozzle installation to determine what K to use for it.  Inward projecting K = 0.78, flush and sharp-edged K = 0.5 and flush and rounded K = 0.04 to 0.28 (consult a reference like Crane's TP 410).
