For a case where there are tension bars doweled into a slab or wall, does the break-out cone for the concrete still exist as a failure mechanism, or is this failure mode for headed studs and expansion type anchors only?

Epoxied bars in tension can have a concrete cone type failure.
See ' Fastening Technology Manual, by Hilti from their web site.

The question comes up because in looking at Hilti's product catalog (4.2.6 HIT RE 500 Epoxy Adhesive Anchor) no mention of concrete breakout is made in the capacity tables. Am I to assume that the values listed for embedment to develop yield or tensile strength are based on the adhesive strength alone?

Have you looked at the ICC report?  The report for the RE-500 SD lists addresses concrete breakout.

Also, can you use RE-500 and not RE-500 SD under your building code?

They might not mention the concrete breakout strength in the product catalog. It does mention the concrete breakout strength in the product technical guide for the HIT-RE 500SD adhesive, which is very similar to the HIT-RE 500 adhesive.  The calculation of the concrete breakout strength would be similar.

The technical guide is very useful and has lots of information that is simple to understand.

absolutely...they'll give you any literature you want if they think you'll be spec'ing their products!

If you have the "Product Technical Guide - A guide to specification and installation", the concrete breakout strength example for the Hilti HIT-RE 500 SD adhesive is in section 4.2.6.7....page 259-260.
