Do any North American codes state at what pipe size two welders should be utilized? Typically after 12" most clients that I have worked for recommend using two welders, however, I am not sure if this is actually in any of the codes. Please reference the code if it is.

No, not with the Codes and Standards I deal with - ASME B&PV Code, Standards and the NBIC.

It's going to differ in the applications = Space, access, fumes and welding fumes removal, time available for the project, separation distance between welders (too small and they're in each others' way and in each others' arc flashes, heat removal and heat rate, movement and expansion of the weld base metal, cost of two welders against time savings of two welders, .....
