Anyone care to explain said table to me?

This whole design guide needs to be redone, IMHO.

For the pullout of strengths of Grade 36 bars it appears the table uses 3 ksi concrete and for grade 55, 4 ksi, grade 105, 5ksi.
But, no where does the guide make mention of this.
Am I missing something here?
Is it standard practice to use Gr 36 and 3ksi together, Gr 55 and 4ksi....etc?
In addition, what is the point of publishing pull out strengths for the concrete that are higher than the strength of the bolts as published in table 3.1

I thought I was losing my mind looking at this table and seeing the bolt Grades on the column headings.
