I am trying to find a good reference text for concrete pole base design (for parking lot lighting).  Does anyone know of any?  I'm sure the key design checks are sliding, overturning and shear.

Are you interested in designing the concrete or it as a foundation?  They're in different parts of the code.  Also the references are different.

I'm interested in designing the base as a foundation.  I will be using 30 MPa concrete delivered to the job site.  I need to design the diameter and depth of the pier and the footing.   I know that I have to check the strucutal aspects of sliding, overturning and shear.  The pole being attached to the base is 12 m in height.  There must be reference texts out there that I can use just having some trouble finding a resource.

Assuming you're in the states, you can use AASHTO or ASCE 7 for wind and seismic. AASHTO is much more conservative for the wind loads.  For the bolts, use ACI 318 Appendix D.  For the foundation depth and width, use IBC Chapter 18.  It's unlikely there's a textbook that's going to cover all this, but maybe someone else knows of one.
As an alternative, go to the Florida Department of Transportation website http://www.dot.state.fl.us/structures/proglib.shtm and download their Mathcad program on high mast pole design.  It's pretty complete.  I wouldn't use it until I had done a manual design initially, but it's a good way to check your numbers.

Try looking at some good geotechnical literature. The pole base actually performs as a footing, only its circular.  Treat it as a circular footing with a direct load being added to it.  There are many equations to use that gives you the demensions of the base granted that you know the applied loads. Look up bearing equations. Since you know your loads(The pole) then solve for the dimensions. Plug and play I say!!!:))))))
