As per UG-14, hollow cylindrically shaped parts may be machined from rolled or forged bars for only the sizes not more than 4". So, does the code implies that the larger sizes are not to be machined at all? If yes, does it mean that nozzle hubs in self reinforced openings shall be forged to precise net shape to be used in vessel?

I think the key word there is "bar".

Also the key word in UG-14(b) is "Except for flanges of all types".

Correct, that limitation only applies to machining a round bar product form < 4"
