Situation,60"  trunk line pipe , there are "butt straps" which are used to connect two 60 " pipes which is connected to the main line. The "butt Straps " and the pipe itself are the same thickness in material. these straps are to be "Full Penetration" welded with a 1/4" backing strip of the same material. The process used is 6010 for root and all the passes thereafter. Although, the specification states , "all field welded double welded Full penetration weld must have , at the root, Dye pen. And once the root passes, the joint  may be complete . Once weld is completed the weld is Radiographic tested". I believe, at the minimum, this joint, per code, should be non - destructive tested. This joint was not tested post completion. One month later , when the pipe was all grouted both in and out, the contractor performed a hydo test and a leak was discovered ( pin hole leak) coming from the full penetration welds. Needless to say , this particular "full Penetration" was not tested. In the ASME Section 9 , does it state requirements, which are directed toward all full pens to be non -destructive tested? If so, where would that be located.  Thank you

Section 9 deals with weld qualifications.  Other sections tell you what specifically needs to be tested.  Welds can be fully radiographed, spot radiographed, or not radiographed depending on usage.  That sounds like more of a contractual issue than a code issue.
