Have a 40 ft span plate girder (riveted) approximately 5 ft deep.  I need to cut a 3ft x 3 ft hole in the web.  Anybody have any references?  The codes only seem to apply to class 1 and 2 rolled sections.

Well, back to first principles.  Where is the 3x3 hole going?  Is it in a shear critical zone?  Can the girder with the hole resist the loading within the code requirements?  If not, hole should be reconsidered.

Have you taken a look at the references listed in the commentary of G8 in the AISC manual?
