I've been trying to make a special toolbox  "bushing", which will mount flush into a hole in a plate.  getting the mate reference to put it in flush is not working for me.  What's the trick?

Without seeing the parts we'd just be guessing.  Coincident and concentric mates don't work?

I've PnG'd the parts & assy as a demo for you.  I want the  cylindrical/7/16-14 od to be in the 7/16-14 threaded hole., with the flat face of the "keensert" to be flush with the top face of the plate.
I've added a "tang" from the face of the keensert for visual purposing only.

Is the face perpendicular to the hole?

Coincident-Flush?
Are you having trouble with direction of face you can hit Tab to reverse direction.

Just took a look at your model. When you select the Circular edge when doing an Alt+Drag smart mate SolidWorks only uses the edge to get the conical face defining the Axis and the flat Face the edge lies on there is no Xpert magic involved.

For your case just add the end face as your second mate ref.
Right Click the Mate ref choose edit Definition
-(icon needed) "come on Context Menu Developers"-
Choose the flat face you want and pick Anti-Aligned.
If you select Any the Tab key will work to flip direction.
Closest will take the direction nearest current rotation.
