Hey all,

I'm writing a corrective action report for a Type XI plate d failure. From looking at the picture in the book it seems that this failure is caused by the weld not being ground smooth after the weld is laid down. Is that correct? Or is it incomplete fusion? I really hate this spec, it's so subjective.

The failure is inside a bolt hole.

How should I address this problem, besides just looking for "ugly weld repairs" and fixing them. The failure is inside a bolt hole on the flange.

Attached is the sample image from the MSS book, in case u dont have it handy.

Visual examination without measurable criteria will always be subjective.  Perhaps you could consider instituting dressing of all repair welds as standard in your quality system procedures as a means of removing the issue?

The plates are intending to show 'excessive' reinforcement and 'excessive' ripple.

But I am wondering how to specifically address this type of discontinuity. Is there a specific action attributal to the welder that would cause this. Not preheating the area to be welded, using damp rods, etc.

As a layman trying to explain this to people even less knowledgeable I'd like to have something more specific than saying "he didn't follow the wps". Even if it's an over simplification.

Most undercut and ripples of repair welds are workmanship versus technical issues related to a welding procedure. You need to have a separate welder instruction that required blend grinding of repair welds in preparation for nondestructive testing. Usually, most experienced welders understand blend grinding and what is required versus leaving repair welds "as deposited".
