i left all my books at my house.. anyhow, am searching the internet for a guide to design embedment length with welded plate at the bottom.. also designing the plate thickness and width

The plate thickness is a function of the bearing stress against the concrete and its dimensions.

The plan dimensions are sized to ensure concrete pullout doesn't happen.  Unless you have an unusual condition, I would just use a nut tack welded to the rod.  Unless you have a sea of concrete on all sides and a large embedment, pullout is not likely to control your design.

If you just want to know.  Size the plate to ensure the factored bearing stress is less than 8f'c*phi, where phi is .65 (I think).

I am sure that if you need an end plate to transfer your forces, then you are dealing with pretty big forces. In that case you might need reinforcing to transfer the tensile force into concrete and not rely only on the cone failure of unreinforced concrete.

In that case your embedment depth will be controlled by the depth needed for the development of a standard hook in tension (ldh), where ldh is calculated from the base of the anchor rod.

ACI 318-08 Appendix D has a good picture showing this.
