Has anybody ever heard of a foam pourer being inserted through the roof of an API 650 built storage tank?

No......I have never heard of a "foam pourer"..

But there is such a thing as a "foam chamber" used to direct foam distribution during a fire event.

The "foam chamber" should be specified in the original tank design. This also means a fire protection study should be done at the time of storage tank specification and purchase....early in the project.

This is something that the MBAs frequently ignore...
