Are the thicknesses listed in Table 136.4 (NDE requirements) of ASME B31.1 the nominal thickness?

No, they are thickness limitations. You can have joints with different thicknesses.

So if I have a .719 nominal thickness pipe and the mill tolerance allowed by the ASME/ASTM spec could put the wall thickness over .750, ASME expects me to measure every end prep to determine NDE requirements?

The information in Table 136.2 of ASME B31.1 is for NPS, which means pipe schedule and wall thickness.
