The anchor has a flat rectangular profile with serrated edges at its free end. It was being welded to the inside of a boiler shell of carbon steel with a Nelson stud gun. The anchor itself was 310 stainless steel. It was intended to hold blanket insulation in place. The procedure was qualified beforehand on a test specimen. Several specimens were bent over 90 degrees and passed. The insulation contractor started to bend the production anchors back and forth about both the weak and strong axis resulting in many failures ( apparently nobody had even requested him to do this). The structural welding standards require bending the round concrete production stud studs 30 degrees. What is the correct test for the insulation anchor? Also should it be annealed ?

Production studs should be bent in one direction only.  Annealing shouldn't be necessary.
