We are repairing a pressure vessel that is UM stamped, there is no National Board number. Is it required to R stamp such a vessel that has not been registered with the National Board? What if it is UM stamped, but has been registered?

Take a look at the Jurisdictional rules at the point of installation....In general, because ASME vessels cross state lines, it is wise in ANY case to apply the R-Stamp for ASME Stamped vessels, otherwise, you would have difficulty locating the appropriate documentation when the jurisdiction has a question regarding the installation.
FYI, R-Stamps can be applied to Non-ASME Vessels as well.

An UM stamped vessel without a NB number was still built per ASME Code and can be repaired per NBIC including a Code Stamping. Moreover, NBIC is applicable not only to an ASME stamped items but to any pressure retaining item, does not matter, it was registered with the National Board or not. You may repair an item built to any foreign code (see NBIC Part 3, page 14).
However, the Code Stamping is required for any repair except routine repairs can be performed without R-stamping, subject acceptance of the Inspector and Jurisdiction, see NBIC page 88.
The bottom line is: you need to call you Inspector to receive his prior authorization for the repair as required by NBIC, 1.3.1 (page 15) and also you need to contact Jurisdiction of the vessel owner-user to verify if they have a specific requirements for repairs of pressure vessels.
