We received a Structural Steel report.  Can someone direct me to a reference that will show the following:
1.  The weld was unacceptable due to overlap.  Overlap
2.  The weld was unacceptable due to being underfilled.  
3.  Excessive undercut.

Assuming this was a report for a building in the US, you would be complying with AWS D1.1, Structural Welding Code-Steel.  This code provides the discontinuities you noted and the allowed amount of each for particular types of welds.  Based on the description, these are likely fillet welds with issues.

As stated above you would want to see AWS D1.1, also AWS A3.0 is a whole book just on welding terms.

Overlap is the protrusion of the weld metal beyond the weld toe or weld root.

Underfill is a condition in which the weld face or root surface extends below the adjacent surface of the base metal

Undercut is a groove melted into the base metal adjacent to the weld toe or weld root and left unfilled by weld metal.
